Original PDF formatting and interrelation of text and graphical elements on each page. Adobe Acrobat is not required.
Version 1.9.9.5 fixes incorrect font rendering, adds option to specify the size of output image and option to crop image. All conversion options can be set separately for each file.

Version: Advanced PDF to Image Converter 1.9.9.5
Pros: I had a pdf file that I wanted to convert into PNG images, one image per page. It did the job rapidly and accurately.
Cons: The program is tooo expensive. Considering that all I wanted to do is convert just one single file to images, I find it hard to justify paying $50 for this. The trial version leaves a water mark on each image. So, I guess this price tag makes it worth the effort trying to remove the water mark using a photoshop macro.
