4LightData Forms provides an easy way to fill out PDF forms. Multiple Templates can be created, each with its own design. Completed forms are stored and can be searched, edited and printed. Numeric entries can be summarized for each form type. Design a Template from scratch with the included drawing tools, drop a PDF or Image File as a background or combine both methods. Place and format entry areas anywhere. When entering data into a form, additional data about the particular form can also be included without being seen on the printed form. Form Templates and even completed Forms can be exchanged between different Forms databases. Send form Templates to other Forms users and be able to receive completed forms from them via eMail.
What's new in this version: Added a Count column and the ability to select a footer formula in the Subtotals result window.
Print and Print Merge will print pictures from embedded database picture fields. Fixed sync problem with picture scrolling.

I downloaded and tried. You have to guess at everything. I've been a Mac User since 1986 and I can't make head nor tails of how to use use
Summary
Would recommend for anyone to use unless they have a PhD in the subject. Only people that can make use.
