From QTam Computer :

QTam Bitmap to Icon is a small program for creating icons. Simply resize your image to 32-by-32 or 48-by-48 pixels and then use this program to convert the BMP to ICO. No other runtime DLL is needed. The program has an autoconvert function and the ability to mask all valid BMPs in the directory to ICOs with one touch. QTam Bitmap to Icon version 3.0 has no limitation on the input picture size and color depth. The last added support for BMP, GIF, and JPEG formats, and gave you the ability to convert multiple files to icons with the automask function. Version 3.1 fixes the black spot bug from the previous version.

Note: In the unregistered version, you can't input GIFs or JPEGs and you can convert only up to three files at a time.
