Summary
I used this app in Word 2003 under XP for years without any problem. Now however it refuses to work in Word 2003 under Vista 64. The installation appears to go ok, no errors, but when it's finished the Avery button simply is not there. Looked in Tools etc. to make sure the right toolbar is enabled, it just won't appear. I'm guessing it's because I'm using Vista 64, I think it says somewhere that it will only work in Windows 7 64-bit, not Vista 64.
