From Nuance Communications:
OmniForm 5.0 enables you to quickly turn paper forms into digital forms, or to create new digital forms from scratch. OmniForm 5.0 comes with a complete set of pre-designed templates to facilitate the creation of new forms. These forms can be printed and distributed, or they can be sent via e-mail so that recipients can fill them out on their computers.
Omniform has been discontinued by Nuance and is no longer sold or supported. If you call Nuance they will refer you to 'Formdocs' which sells a similar form-design and form-filling software with many of the same features as Omniform. As of 01/2013 Formdocs was named by Nuance (the makers of Omniform) as the official replacement for Omniform. Formdocs welcomes all Omniform customer enquiries and is happy to answer any questions about migrating from Omniform to Formdocs.

VERY powerful form generation, either "from scratch" or from scanned in files. Convert existing forms, or things which are not really forms, into "live" forms.
Hasn't been updated in years. Lacks some "tools" you might expect in a more current Windows application. Requires some patience to learn to use.
I haven't found a "better" tool for doing what Omniform does. So, even with it's lack of certain conveniences, It's may be the best available tool (unless you have a huge budget for "corporate" applications.) It is NOT "buggy", it just lacks some 'automatons' that would be useful. An UPDATE would be a very good thing.
