From Innovative Music Systems :

IntelliScore WAV to MIDI converter listens to WAV recordings and helps figure out the notes, chord names, and key. IntelliScore recognizes polyphonic music, that is, music containing several notes at the same time, such as chords. You can record your music directly into IntelliScore, or specify any standard WAV file. The generated MIDI file is suitable for play back, notation, and editing. With this program you don't need a MIDI-enabled instrument or know how to play an instrument to create MIDI files from your favorite music.

Note: Recognition is limited to the first 15 seconds of the WAV file and only nine instrument filters are included.
