From Bebosoft :

Create custom PHP, ASP, and Perl scripts for HTML forms and send the field values thru email, in text, html or multipart, with file uploaded in the forms and then attached to the email, or dump the field values to a text file, MySQL or MS-Access database. Validate your user input for correct values, like correct email, length, number range, credit card number, missing value. Send confirmation email, with file attach and field values. The script is created and customized for your HTML form, which means is very secure against spammers, but includes the ability for HTTP referer and IP address filter. Version 2.6.4 fixes a redrawing problem of the main window, introduced in version 2.6.3.

Version: Forms To Go 2.6.4

"Easy and Excellent"

Pros: I bought this software and since then I don't use any other software/script for my web based forms. This software is easy to use, has many useful features, has a good documentation and a great customer service. I really recommend this software and no, I don't work for them.

Cons: The only con is that the interface seems too simple for what the software does. The latest version is even more simple.
