From Direct Logic Systems :

CoverPro was created to easily print CD covers and labels. A single click will load a CD cover or label image. Then select the type of cover you want to print. CoverPro will automatically adjust the image to the proper size. You can fine tune and customize the print size and print position if desired. You also get five user-defined slots for custom covers and labels for things like VHS tapes, floppy disks, or other media. CoverPro 7.1.0 now allows you to rotate the print-out 90 degrees so you can fold the front cover over to make it a double sheet. Cover Pro is not crippled in any way, and registration costs $9.95.
