Create and fill medical information sheet in English, Spanish, French, or German.

This program uses Microsoft Excel to create a one page Personal Medical Sheet, a wallet card, and emergency information cards (for display and to carry). The titles on each form are available in four languages (English, Spanish, French, and German) any of which can be customized to match individual preferences or needs. The language can be changed at any time to make entering information or producing reports most useful. Information possible and prompted for (but not required) is Personal (birth, blood, telephone, email, address), Emergency (critical conditions, allergies, contact details), Insurance (primary and secondary company, policy numbers, telephones, addresses), Allergies and Sensitivities (cause and effect), Medications (name, dose, number taken and when, purpose, prescribing doctor or over the counter), Medical Personnel (name, specialty, telephone, address), Medical Conditions or Concerns, Medical History (date, event), and room for miscellaneous information. Any of the information can be entered or changed at any time and current sheets/cards printed. The report formats can be altered as desired (font color, alignment. etc.). All of this is done privately in the comfort of home or office, and will then be readily available for any medical professional when needed.This one download can be used by the entire household.
