Server installation. This is more for large companies.

Pros
It seems it is an advanced / enterprise product. The file size is like 40MB
Cons
I have not tried it yet, but when I saw that it needs to be installed on a server then it is probably not for me because I am a single user with no server yet. They provide a basic license that will provide a summary report and a comments report.
Summary
When you click on the download link they ask you to register with them. I had to fill out a contact form and they sent me the link to download the software. I did not like that I was limited to a summary report and a comments report. If I want to have no limits I would have to pay $250 for a 3 user corporate license and they have other licenses that can cost $2400 and $4500. They also have a hosting service at $800 and $2950 per year that I suppose is to allow to collect surveys at their site. Based on all of the above I think this is not a product for something small where you just wanted to fill out some forms at home for your own use at home, not to collect information on a server from many people like this product probably does.
