Primary Health Care Revitalization Division (PHC-RD) is one of the divisions of Department of Health Services, Ministry of Health Nepal.

PHC Revitalization Division is one of the divisions of Department of Health Services which is responsible to revitalize PHC in Nepal by addressing emerging health challenges in close collaboration with the other divisions and different supporting actors. The division is also responsible to make inroads into translating the constitutionally stipulated fundamental right of basic free health care into practice by addressing the disparities in health service delivery and promoting equitable health services.

The office of PHC-RD is located at Teku, Kathmandu.
