Mafia: The Old Country is a third-person action-adventure game centered on the rise of organized crime in early 1900s Sicily. The story follows Enzo Favara as he enters the Torrisi crime family and becomes involved in territorial conflicts, criminal operations, and family obligations. 

Mafia: The Old Country combines Narrative Campaign progression with combat, exploration, and mission-based objectives across rural and urban locations. A Period-Authentic Setting drives its environments, weapons, vehicles, and social structure, while Combat Systems incorporate firearms, stealth actions, and close-quarters blade encounters. Travel occurs through horseback riding, on-foot movement, and early motor vehicles as missions unfold.

Mafia: The Old Country story advances through scripted chapters that place Enzo within the hierarchy of a Sicilian crime family. Dialogue sequences, character interactions, and cinematic scenes establish relationships between allies, rivals, and family leaders. Environmental variety forms a major part of progression, with missions moving across both rural and urban regions. Exploration focuses on narrative destinations and mission routes rather than an open-ended sandbox structure with extensive side-management mechanics.
