The Care Navigator Mobile App provides a means of accessing and updating information in the Care Navigator Portal for on-the-go care team members and patients. Care team members and physicians are able to engage more closely with their patients given the capability to view and update patient information, care plan summaries, appointments and alerts at the touch of their fingertips. Patients also have more control of their day to day functionalities with treatment calendars and appointment reminders.
