The reason why this program caught my attention was its size. How can a 574K program in these times do a job? Surprise! Surprise! The engine works, and it works good.
Try out MXLookup=1 option. It is not mentioned very explicitly in the documentation, but it will do a no-nonsense spam relief.
If you are a GUI lover, out of the box software user, this product is not for you. There is no installer included, you don't need one. It does not tinker with the registies. You have to read the installation notes of the user guide to configure the product using INI file and a bunch of text files.
