  Here is one that does everything you need, and it's based on a Star Rating System that derives the appropriate tip percentage and is fully configurable to your preference. The app name is "Rate My Tip".

  Pros

  Simplicity and Free are good. It gets the job done.

  Cons

  Other features such as splitting among drinkers and non-drinkers, tax, tip rates are missing. The design is not attractive.

  Summary

  For just $1 you can buy another app called TipBox (also in AppStore) for the real use. I'd recommend TipBox since it has beautiful interface and all the functionality in place. If you're just trying the tip calculator out, either one's fine. There are dozens of tip calculators out there.

  It definitely does the job. It is simple and works easily.

  Cons

  It isn't very sexy looking and it doesn't really do anything unique.

  Summary

  If the front end user interface was polished a bit I think it would be better. Some of the new iPhone tip calculators are pretty cool. I switched to Magic Happy Pro Tip ($2 in the Apple AppStore) after finding most of the free ones to be of limited usefulness. Pro Tip looks good and works really well, it even has some cool features that I wouldn't have thought of but are quite useful.
