Email Questionnaire is an interactive questionnaire system that is e-mail-based. With our innovative WYSIWYG editor, several styles of templates and the questionnaire library, the survey design is easy and quick. You can use your own logo and organization-style template. The mail merge feature combines the questionnaire with user information instead of "Dear customer" form letters. Email Questionnaire version 5 provides you the new send format options: PDF and Flash survey form in addition to MIME encoded message and HTML. PDF format is cross-platform and has good printing support. The ratio of successes to submission can be raised efficiently. The software works by using your existing e-mail system to make on-line surveys available to anyone with an Internet connection. Recipients' replies can be retrieved automatically from a mail server or the existing e-mail client. No reply e-mail opening is required. And the reply e-mail is plain text and size is small.

A convenient tool to create a questionnaire and do the statistics.

Pros
Easy to create a questionnaire in html format and I don't need to write the html file. The tool can submmit it by email, so the receivers can preview it in outlook express or outlook and just click the selection bottoms to reply this questionnaire.
Cons
It is better to create questions or selection items by clicking icons instead of by choosing from the menu.

Makes managing questionnaires really easy.

Pros
Sending questionnaires and replying all via email is really a good idea. Managing past questionnaires/viewing reports is simple.

A relatively complicated and in my opinion inadequte piece of software that I very much regretted buying. It did not do what I wanted it to do and did not work when sent to Yahoo accounts (not many of those eh?). It did not list its shortcomings
