"does what I need it to do"
Version: FormulatePro 0.0.6
Pros
Allows you to add and fill text boxes in any PDF
Cons
I can't figure out how to erase or cover over text already entered in a pdf
Summary
I was a bit wary of this software had only two reviews, one of which was not good. But for my purposes, it did what I needed it to do pretty quickly and easily. You have to print to PDF rather than Save as PDF, but that's not a deal breaker for me.

"Not very good"
Version: FormulatePro 0.0.6
Pros
On Mac OS X 10.6.8 the type, square, ellipse and pen tools work.
Cons
The marquee tool does not work. The pdf that is downloaded from Formulate Pro won't load in Skim or Preview. There is no Help. I find it erratic.
Summary
Sometimes allows one to enter text on a pdf, sometimes not. Which can be irritating. It has not been upgraded since 2007 and has no Help.

"Change the search tag on this one!"
Version: FormulatePro 0.0.6
Pros
Simple, easy to use, small footprint---just what I needed!
Cons
Change the search tag on this one.
Summary
I have been looking for an app for my mac to fill out scanned .pdf forms ever since college. I found this one using "pdf form" instead of my usual search criteria. Add some more intuitive tags for us simple folks!
