CNET Editors' review
People who use a lot of files in Adobe's Portable Document Format (PDF) but can't afford one of Adobe's high-end products often end up buying third-party utilities designed to make creating and managing PDFs easier, which they do, but since many cost $50 or more, it quickly becomes an expensive proposition. Fortunately, software maker Some PDF has several high-quality PDF utilities that have the added advantage of being totally free. One of them is PDF Image Extract, which extracts images from PDF files without having to cut and paste and saves files in a wide range of formats, including JPEG, GIF, TIFF, BMP, and PNG.
Image Extract downloads, installs, and opens quickly and gets right down to business. A simple, well-thought-out interface makes it easy to load your PDFs, select your output options (such as image quality, gray scale levels, and other settings), and perform the extraction. You can import single files or whole directories. Tabs on the main window's toolbar let you enter the owner's and user's passwords.
The program works well, which is just as well, since the help and documentation features are sparse and often seem to have lost some clarity in the translation to English. Online help is available, though, and Image Extract is easy enough to figure out without much guidance. Some PDF Image Extract is freeware, but for donations of $20 and up the developer will automatically notify of new releases.

"Textless images from a layered pdf - this is it."
Version: Some PDF Image Extract 1.5
Pros
Extracts all images from the indicated page(s), then converts to the designated format. Reliable. Extracts images from the image layer only, removing text and watermarks from other "layers" in the .pdf.
Cons
Occasionally would hang up on a second batch of extractions - simply close and open again (small program, minimal effort). For the value, this is barely worth mentioning.
Summary
I searched for weeks for an application to extract a textless image from a .pdf. Adobe wants to charge $50+ for a license to Adobe Acrobat. Photoshop will do it, too, but that's even more expensive. Several friends understood my issue and offered some hacked software to do the job, but I wanted a legit, repeatable method.
After several hours on a weekend morning, my GoogleFu brought me to CNET and this application. It did exactly what I wanted:
Extracted the image I was after,
Did not include the text-layer overlays on the image,
(bonus) output the image in the graphic format for my choice.
I'm a gamemaster for a gaming group, and I am extracting "maps" from adventure-bundle .pdf's. These maps frequently have GM-only notes, room markers, and "secrets" labeled, making use of the Snapshot tool useless. Any image created with the snapshot tool in a .pdf viewer would include these markings, which the players are not supposed to see. This program quickly, cleanly extracted JUST THE TEXTLESS IMAGE to a usable file.
If you've been searching for as long as I was, and you end up here, your journey is over. Use this program to get the images you want. -
