Link is the Stanford Childrens Health home monitoring CareKit application. When Link is prescribed by a provider, you can authorize data entered in Link to be visible in the Stanford Childrens Health electronic health record for review by clinicians. This integration is achieved through HealthKit and the Epic MyChart mobile application. The initial scope for clinical data to be shared includes vital signs (e.g. heart rate, oxygen saturation) and weight, and is designed to automate the flow of data for the Stanford Childrens Health Single Ventricle Home Monitoring Program. The app also includes the ability to create a daily medication checklist, review data trends, and store contact information for providers and other caregivers. It was designed to improve the patient experience and is integrated into provider workflow to enhance clinical care.
Care Contents Screen Summary:
The Care Contents screen includes the medication checklist and the area to enter vital signs (heart rate, oxygen saturation) and weight.
Medication Entry Screen Summary:
The Medication Entry Screen allows for configuration of the medication and dosing schedule that populates the medication checklist.
Insights Screen Summary:
The Insights screen displays line graphs to trend clinical data entered over the past week.
Contacts Screen Summary:
The Contacts screen is pre-populated with important contact information and lists patient-entered contact information if desired.
Contact Entry Screen Summary:
The Contact Entry screen allows patients to enter and edit contact information that displays on the Contacts screen.
